After Dan's suggestion, I took a close look at FXPrimus and it appears as though they meet the requirements.

I have set up a demo account which is only active for 60 days. The demo account is http://www.myfxbook.com/members/rsme...xprimus/171277

Note that I made a mistake setting the opening balance and accidentally set the balance to 500K instead of 50K. The initial lot size has been set to one standard lot (1.0) in line with the 5K per micro (0.01) formula.
